Who May Request a Certificate of Birth Resulting in Stillbirth?


A parent may obtain a Certificate of Birth Resulting in Stillbirth by completing an Application for Certified Copy of Death Record. Parent is to write “Certificate of Birth Resulting in Stillbirth” in Part 2, Intended Use of Certified Copy.

 

Since the name of the stillborn fetus was not included on the Certificate of Fetal Death until 2006, the mother or father may provide on the application form the name of the stillborn fetus to be listed on the Certificate of Birth Resulting in Stillbirth, provided that if a name is given it must be the same as the name on file with the Department, if any. 


 

Cost


The first copy is available at no charge. There is a $9 fee for each additional copy requested. This fee may be waived for the dependent of an individual who served or is currently serving in the Armed Forces.

 


How to Get a Certificate of Birth Resulting in Stillbirth


There are 2 ways to obtain a Certificate of Birth Resulting in Stillbirth. Be sure to complete all required information on the application form provided within each link:    

 

1.       Request by mail (Note parent is to write “Certificate of Birth Resulting in Stillbirth” in Part 2, Intended Use of Certified Copy.)

2.       Certificate of Birth Resulting in Stillbirth applications may be filed in  person at a public office; however, the Certificate of Birth Resulting in Stillbirth will be mailed from the central office in New Castle, Pennsylvania. (see 6 locations below):
